Parents/Guardians requiring proof of enrollment (i.e., CRA, CIC, Citizenship, etc.) for their student(s), please complete the release of information form and email it to Ms. Davies at firstname.lastname@example.org, There is a cost of $20 and may take 3-5 days to be ready for pick up at the office. If there is any financial concern about this, please let us know.
STUDENT MISSING EXTENDED SCHOOL TIME – Please complete the Student Missing School Time Form, if your student(s) are going to be away from school due to an extended vacation, or a family leave

Field Trip/Event Volunteer Drivers – bring student(s) booster seat if required.
To protect our children and you as a driver, we ask that you complete the following. You will need to provide the school a copy of your driver’s license, driver’s abstract and your current Autoplan Insurance Policy, the policy must include a minimum $2 million liability insurance.
If you are able to drive please bring copies of the following to your students teacher. The office does not have time to copy documents for families
- Volunteer-Driver Form
- Copy of driver’s license Class 5 (NO “N” or “L” )
- Copy of drivers abstract will need “Personal driving record” https://onlinebusiness.icbc.com/clio/
- Copy of the first two pages of their car insurance and they must have $2,000,000 liability. One page has vehicle information, second page will have insurance coverage.
All forms will be shredded at the end of the school year, as people’s insurance and licenses may expire
Medical Concerns –
Please make sure that you have indicated any medical concerns or information that the school should be aware of (e.g., asthma, allergies, etc. please complete medical forms if needed)

Aboriginal Ancestry is determined on a voluntary basis through self-identification. This includes First Nations (Status or Non-Status), Metis or Inuit Ancestry. No documentation other than this self-identification is required and the ancestry can go back several generations.
